The dog and cat applications that we have online are NOT interactive forms.

You can either print the web page from your browser, or download and edit the file. If you print out the page, you can then fill it in by pen and bring it with you to an adoption show. PLEASE NOTE: an application sent by mail can take two weeks to get to the correct PAW person, so please contact us by email or phone instead!

We have also provided the applications in both MS Word format and pdf formats. The following steps should help you prepare the form:

• Click on either the pdf or MS Word version of the form below.
• Click on the "File" menu and choose "Save as..."
• Remember where you saved the files for later e-mail.
• Edit the form on your computer.
• Save the final version of the form back to your computer.
• Print or email the saved version to PAW for approval.
